FDA Prior Notice Data Entry  
 

As a function of the FDA Bio-Terrorism Act, it is required that shipments being imported into the United States containing FDA controlled goods notify the FDA prior to their shipment and entry into the United States. Use this screen to submit FDA Prior Notice (PN) Numbers or create a FDA Prior Notice (PN) for your shipment.

To submit FDA Prior Notice (PN) Numbers
    1. Fill in the appropriate fields:

    • Type
      Select the type of submission:
      • New - The first submission of the information required for UPS SCS to obtain FDA PN Numbers on your behalf, or inform UPS SCS about the FDA PN Numbers your have already obtained from the FDA.
      • Update - Request a previously submitted FDA Prior Notice be updated with new information (Original Confirmation No. is required).
      • Delete - Request a previously submitted FDA Prior Notice be deleted (Original Confirmation No. is required).

    • Header Information
      You must complete all fields with the * beside them, otherwise FGV will not allow you to submit your FDA PN numbers.
      • Schd D Arrival Port - You must enter a valid Schedule D Port Code in this field - if you do not know the Schedule D code, please use the Look Up function to determine the valid code for the port of arrival.

    • Request Details
      You must complete all fields with the * beside them, otherwise FGV will not allow you to submit your FDA PN numbers.
      • Unit of Measure (UoM) - You must enter a valid UoM in this field - if you do not know the UoM code, please use the Look Up function to determine the valid code for the unit of measure.
      • FDA Country of Production - You must enter a valid ISO Country Code in this field - if you do not know the ISO Country code, please use the Look Up function to determine the valid code for the country.
      • FDA Prior Notice Number - You must submit the FDA PN Number when using this screen - if you do not have a FDA PN Number for the line detail, you must create a FDA Prior Notice.

    2. Use the edit buttons to Add More Line Details, Save, Cancel or Reset transaction contents.

    • Add Five More Line Details - Adds five more blank line details.

    • Save - Click Save to complete the submission of the FDA PN Numbers. Upon successful submission of our FDA PN Numbers, a confirmation page appears and provides you with your Confirmation Number for the transaction.

    • Cancel - Click Cancel to return to the Login or Home Page. Do not click Cancel unless you want to Cancel the transaction.

    • Reset - Click Reset to clear all the fields you populated on this screen. Do not click Reset unless you want to clear the screen.


To Create FDA Prior Notice (PN) Numbers
    1. Fill in the appropriate fields:

    • Type
      Select the type of submission:
      • New - Submit information for the first time.
      • Update - Request a previously submitted entry be updated with new information (Original Confirmation No. is required).
      • Delete - Request a previously submitted entry be deleted (Original Confirmation No. is required).

    • Header Information
      It is required that all fields with the * beside them are completed, otherwise FGV will not allow you to submit your FDA PN numbers.
      • Submitter Firm Type - Select the appropriate firm type from the drop down menu.
      • Schd D Arrival Port - You must enter a valid Schedule D Port Code in this field - if you do not know the Schedule D code, please use the Look Up function to determine the valid code for the port of arrival.
      • Country Code - It is required that you enter a valid ISO Country Code in this field - if you do not know the ISO Country code, please use the Look Up function to determine the valid country code.
      • Country of Shipping - You must enter a valid ISO Country Code is in this field - if you do not know the ISO Country code, please use the Look Up function to determine the valid country code.
      • Carrier Country - You must enter a valid ISO Country Code in this field - if you do not know the ISO Country code, please use the Look Up function to determine the valid country code.
      • Mode of Transport - Select the appropriate Mode of Transport from the drop down menu.
      • Estimated Arrival Date - Use the drop down menus to enter the appropriate Estimated Arrival Date and time.
      • Container No. - If a containerized Mode of Transport is selected, you must fill in this field.
      • Rail Car No. - If Rail is selected as the Mode of Transport, you must fill in this field.
      • Vehicle Country - If Auto is selected as Mode of Transportation, you must enter a valid ISO Country Code in this field - if you do not know the ISO Country code, please use the Look Up function to determine the valid country code.
      • Vehicle State - If Auto is selected as Mode of Transportation, you must enter a valid state code in this field.
      • Private Vehicle License - If Auto is selected as Mode of Transportation, you must enter the vehicle license number in this field.

    • Request Details
      You must fill in all fields with the * beside them, otherwise FGV will not allow you to submit your FDA PN numbers.
      • Unit of Measure (UoM) - You must enter a valid UoM in this field - if you do not know the UoM code, please use the Look Up function to determine the valid code for the unit of measure.
      • FDA Country of Production - You must enter a valid ISO Country Code in this field - if you do not know the ISO Country code, please use the Look Up function to determine the valid country code .

    2. Use the edit buttons to Add More Line Details, Save, Cancel or Reset transaction contents.
